Credit Card Payment Authorization Form

C&J Express (dba Sea Gull Holiday) will try its utmost to ensure the security of your credit card transaction. In order to guard against credit card fraud, we need you to provide the following supporting documents and proof:

• Photocopy of the valid ID of the credit card holder (valid ID includes US-issued Driver’s License, Photo ID or National Passport with signature page)
• A filled-out and dated Credit Card Payment Authorization Form signed by credit card holder (click this link to download the Authorization Form).

If the credit card holder is different from the customer joining the tour, please attach a photocopy of the passport of the customer.
